Welcome to the new patient center for Humble Chiropractic!
We understand that arriving at a chiropractic office for the first time can be nerve racking. If you've never experienced chiropractic care before, you may not know what to expect when you come to our office for the first time. Filling out paperwork, learning new faces, and getting to know your way around the office can be an overwhelming experience.
That's why at Humble Chiropractic our aim is to remove any unnecessary stress or tension and make you feel comfortable during your visit with our chiropractor.

What to Expect

Patient Forms
Upon entering our office, our receptionist will  welcome you as a member of our family.  We will request that you complete our  patient forms which can be downloaded below.. This paperwork provides us with your health history and information on your condition.
Next, you will have a consultation with Dr. Humble to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr.Humble to learn more about you, your condition, and  expectations to determine how chiropractic care can meet your goals.
After your consultation, Dr. Humble will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. Depending on your specific condition, we may take X-rays as well. X-rays help us develop  the most effective treatment plan for you, and alert us of  any serious spinal conditions.
Report of Findings
Once the information is collected  and examinations are performed, Dr. Humble will give you a detailed report of all findings and answer any questions including:
How can you help me?
How often do I need to come in?
What  will  my treatment  cost?
After reviewing your health history, goals, and examining your spine and X-rays, Dr. Humble will discuss recommendations and notify you if  your condition requires care with other providers. Dr. Humble will provide the best treatment and  wellness program for your needs.
At the conclusion of this initial  appointment, you can choose to  experience your first treatment. This may include spinal adjustments, physical therapy, and/or soft tissue massage. Treatment is interactive, so  you can express concerns about the different styles of treatment  anytime.

  • Verification of the injury from a superior at your employer (only if you've been hurt at work)

Humble Chiropractic offers our patient forms online so they can be completed in the convenience of your own home or office, before you even arrive to our chiropractic office.

How to Pay
 Our chiropractic office participates in many insurance programs, but please call us if you have any questions about whether you can use your insurance to pay for your chiropractic care.

We also accept auto accident and workers compensation cases that result in back pain, headaches or other symptoms which can be addressed with chiropractic care.

For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged.
